1. Valid Photo ID: A passport or driver’s license is the most commonly accepted form of photo ID. The ID must be original (not a photocopy) and clearly show your full name and date of birth.
2. Booking Confirmation: You should bring your test booking confirmation. This could be in the form of an email or printed document, or you can show it on your mobile device. It’s essential for the test center to verify your appointment and identity.
Additional Notes: Ensure that you arrive on time for your test. Late arrivals may not be allowed to take the test and could risk losing their booking fee.

No Valid Photo ID: If you don’t have the required form of photo ID (e.g., passport or driver’s license), you might apply for a WAIVER FROM. However, you would need to provide an explanation and supporting evidence as to why you're unable to obtain this ID.

This ID Waiver Form is only to be used in exceptional circumstances where you cannot provide the required Primary or Secondary ID needed to sit your test. If you're under 16 and need to take the CITB Health, Safety, and Environment Test, the Parental Consent Form is the key to getting started. By ensuring that your parent or guardian provides approval, you can sit the test and take an important step toward building your career in construction.
If you need help with the form or have any questions about the process, don’t hesitate to reach out to us. We're here to support you on your journey to success.

The Parental Consent Form is designed to ensure that a parent or legal guardian is fully aware of and agrees to the minor (under 16) taking the CITB HSE Test. Since individuals under 16 are considered minors, it’s important for a guardian to give their approval before the test can proceed.
This form ensures that the test provider has obtained permission from a responsible adult, allowing the minor to take the test. It also confirms that the parent/guardian understands the purpose of the test and the legal responsibilities involved.

Fill Out the Form: The form requires details about you (the candidate) and your parent or guardian. This typically includes:
1. Your full name, age, and date of birth.
2. The full name and contact details of your parent or guardian.
3. A declaration where your parent or guardian gives their consent for you to take the test.
4. Sign the Form: Both you and your parent/guardian must sign the form to confirm consent.

Submit the Form: Once completed and signed, you will be required to bring a physical copy on the test day.

Supervision on Site - If you're under 18, there may be limitations on the types of work you are allowed to do on construction sites, even if you pass the CITB HSE Test. It’s important to consult with your employer about the tasks you can legally perform.
